MEDOXIS will recognize your old Data-File, so each time you upgrade, the upgrades will be made to the database structure automatically without your intervention. This way you will be profiting all the new functionalities with no worry.
So, each time you upgrade MEDOXIS, please make a backup or a copy of your Data-File, later after re-installation you point the Data-File Location to your working file.
